Collection of your Personal Information

JADE Wellness Center collects and maintains Protected Health Information (PHI) necessary to provide you with healthcare services. This includes information about your medical history, treatment plans, and any health services you receive.

We use and disclose your PHI to:

• Provide, coordinate, and manage your care and treatment
• Communicate with other healthcare providers involved in your care, including hospitals, labs, and specialists
• Process payment for services rendered, including billing insurers or Medicaid

Our Legal Duties

• JADE Wellness Center is required by law (including the federal Health Insurance Portability and Accountability Act, or HIPAA) to maintain the privacy and security of your PHI.

• If there is a breach of your unsecured PHI that compromises your privacy or security, we will notify you promptly as required by law.
• We reserve the right to update this privacy statement as needed and will notify you of significant changes.

Consent for Telehealth Services

• When you receive services through telehealth platforms such as Doxy.me, you will be asked to provide informed consent acknowledging the nature of telehealth, including potential risks and benefits.
• JADE Wellness Center uses secure, HIPAA-compliant technology to protect the confidentiality of your telehealth sessions.
• You may withdraw consent or refuse telehealth services at any time without affecting your right to future care.

How We Protect Your Information

• We implement physical, administrative, and technical safeguards to protect your PHI from unauthorized access or disclosure.
• Access to your health information is limited to authorized personnel who require it to provide care or perform administrative duties.
